Community Utility:

Staff assists consumers to identify health and/or social needs and then provides appropriate referral to the proper entity.  We know our community best and are able to assist those with needs in accessing services to maintain or attain optimal health.  Agency staff may meet or communicate with consumers and/or their support systems several times to identify needs and provide information about community entities that will assist them with their needs.

 Resource Navigation:

Staff provides educational information about physical, behavioral, environmental, social, economic, and/or other issues affecting health.  Staff assists the client and/or support systems to work through barriers that hinder health improvement.  Agency staff may meet to communicate with client and/or their support systems on an ongoing basis to identify needs and provide information about community entities that will assist them with their needs.  The program provides information about access to services (i.e. insurance options, housing, domestic violence, food, mental health, substance abuse, and parenting) that impacts overall health status.  The program ultimately helps clients navigate resources and services and explore options within those systems

 Who is eligible?
Referrals are accepted from any interested party for families residing in Calhoun County.

What is the fee?
Many services are provided without charge.

Services may be fully or partially funded by state and federal grants, as well as health insurance plans.
