Birth Records
Welcome to Calhoun County Vital Records.
Birth Certificates for travel, employment, school, sports, social security, etc. To obtain a raised seal, certified copy of your birth certificate, you can write to us or come into the office. We can process all birth applications in the state back to 1986. Before that you can contact the county of your birth. We also can forward your application to that county or state vital records.
For the records we do not have, you may contact the Iowa Department of Public Health. You can call them at (515) 281-4944.
You may request a copy of county Vital Record form. Apply by mail or in person. A fee of $15.00 is required for each request. Cash, check, money orders, and credit cards are accepted. Please call for more information: 712-297-8121
